• All EQUIP groups who will be staying on-site at Lincoln Lake Baptist Youth Camp, Woodland Christian Camp, Camp Eden, Higher Grounds Camp, Awanita Christian Camp, South Mountain Christian Camp, Shiloh Bible Camp and Retreat Center, regardless of age.

• All EQUIP groups who will participate in supervised and unsupervised recreation, regardless of age.

I give permission for my group to attend the EQUIP 2023 Retreat. I understand that each individual in the group should be covered by health insurance coverage, that information must be presented by each individual prior to attending camp.  
I hereby release and agree to indemnify EQUIP 2023, its agents, Lincoln Lake Baptist Youth Camp, Woodland Christian Camp, Camp Eden, Higher Grounds Camp, Awanita Christian Camp, South Mountain Christian Camp, Shiloh Bible Camp and Retreat Center, and all affiliated church organizations from any and all liability for injury arising out of my group’s participation in this retreat. *

Pay Online: You will be redirected for payment after completing form.  

A non-refundable $50 deposit is required for each participant in order to reserve your spot. Space is limited. The remaining balance is due by no later than May 13, 2023.

*Cost for Lincoln Lake Baptist Youth Camp is $235.
